Code Huddle (Pvt.) Ltd, Office # 202, The Marina Commercial, Cornice Road, Bahria Town Phase IV, Islamabad, Pakistan, is responsible for information submitted through the Physio assessment.
What we collect
When you submit the Physio clinic assessment, we collect the information you enter: your name, work email, optional clinic name, and the operational figures included in your calculator snapshot.
Please do not submit patient names, medical records, health information, or other sensitive patient data through this form.
How we use it
We use the information only to review your clinic’s workflow, respond to your assessment request, arrange a conversation, and prepare relevant commercial or technical follow-up.
Submitting the assessment does not add you to a marketing mailing list. We do not sell the information you provide.
Lawful basis for UK and EU visitors
We process an assessment request to take steps you ask us to take before a possible engagement and for our legitimate interest in responding to a business enquiry. We limit that processing to the information needed to understand and answer the request.
Storage and service providers
The form sends your enquiry through our configured email-delivery provider. If direct delivery is unavailable, your own email application opens instead. Our email, hosting, and delivery providers may process the enquiry on our behalf under their applicable data-processing and transfer safeguards. Access is limited to people who need it to respond to or support the enquiry.
How long we keep it
We retain enquiry records only while they are needed to respond, manage a potential or active engagement, resolve disputes, and meet applicable legal or accounting obligations. Records that are no longer needed for those purposes are deleted or anonymised.
Your choices and rights
You may ask us to access, correct, delete, restrict, or provide a copy of the personal information associated with your enquiry. UK and EU visitors may also object to processing based on legitimate interests and may complain to their local data-protection authority.
